Order online or by phone Order by mail Mail to the Office of Vital Statistics and Registry, P.O. Box 370, Trenton, NJ 08625-0370.
Walk-in center in Trenton 140 East Front Street, Trenton, Monday to Friday, 8:30 AM to 4 PM, except state holidays. After 3:30 PM you can only drop off a filled-out application, and orders are not picked up at the walk-in center.
